Heterotopic pregnancy is that entity in which intra- and extra-uterine gestation coexist. The incidence is low in spontaneous pregnancies; however, it has increased with assisted reproduction techniques. Its diagnosis requires high index of suspicion and the consideration of risk factors. We describe the characteristics of a case of a 26-year-old patient with 9 weeks amenorrhea who came to the Obstetric Teaching Hospital in Guanabacoa, Havana, Cuba. This patient presented with intense pain and ultrasonography diagnosis of hemoperitoneum. She underwent exploratory laparotomy where the gravid uterus and bursting of the right horn of the uterus were confirmed. It was not possible to perform hemostasis, so total abdominal hysterectomy was performed without adnexectomy. The anatomopathological diagnosis was heterotopic pregnancy.
